Sailor Cosmos appears in Volume 18 Act 52 of the orignal Japanese Mamga. She is the last incarnation of Sailor Moon. Some people have dissagree with this statement but the proof is out there.
Sailor Cosmos is one of Sailor Moon's incarnations, which she had many, she was Princess Selenity, then Usagi, then she becomes Super Sailor Moon, Eternal Sailor Moon, then Neo-Queen Selenity, and finally she becomes Sailor Cosmos.
Cosmos comes back to the past (our present)in the form of Chibi Chibi to befriend Usagi, protect her, and help her make the right choices. You see, when Sailor Moon is Cosmos she is faces great death and destruction. All of her loved ones are taken by Sailor Chaos. Facing all of this, Cosmos decides to go back to her past, when she was Eternal Sailor Moon and help her past self defeat Chaos in the battle of the Galaxy Cauldron. But Cosmos does not go as her present form; instead she becomes Chibi Chibi. The reason she cannot go back looking like herself is because she looks practically like Usagi and since during the last three mangas all of these new senshi are showing up and turning out to be enemies. Sailor Cosmos could be thought to be an enemy. But, a sweet, innocent little girl like Chibi Chibi would be protected and yet be near everything to change the past.
In the Manga Volume 18 Eternal Sailor Moon learns that the villain Chaos has been absorbed into the Galaxy Cauldron, the place where all of the other senshi and all of the stars and soliders are born and return to. Sailor Cosmos wanted for Usagi to destroy the Cauldron because Chaos was in it. That way Chaos could not be reborn to wreck havoc and kill everyone in Cosmos' own time. But Usagi said that she could not destroy the Cauldron because it would mean the destruction of everything. So Sailor Moon chose to let Chaos remain in the Cauldron. This is the mistake that Sailor Cosmos wanted to rectify. But when faced with her past self's reason again, she realized that what she had done back then as Usagi was the right decision all along. Sailor Moon had the courage to stand alone so Cosmos could not destroy the Cauldron and she knew she had to return to the future.
